Scheduled weekly check. Inspected recently modified documents across the network, focusing on design stories in z6MkuBbsB1HbSNXLvJCRCrPhimY6g7tzhr4qvcYKPuSZzhno/design/stories/ and supporting documents in /notes/.

High Priority

1. ⚠️ emoji prefix on two design story titles (second week)

Problem: Both ⚠️As a User, I want to create a Site for the first time and ⚠️ As a user, I want to search documents, conversations, text blocks, and media use ⚠️ as a title prefix. The createsite doc additionally has a missing space (⚠️As instead of ⚠️ As). This was flagged for the search doc in the 2026-07-29 report but remains unresolved and now affects two documents.

Why it matters: Emoji prefixes add noise in search results, query blocks, and embedded card views. The ⚠️ already appears as a status indicator within the parent hub pages; duplicating it in the title is redundant. Other design stories (e.g., authoring, identity, conversations) do not use emoji prefixes, making these two inconsistent.

Recommendation: Remove the ⚠️ prefix from both titles. Replace createsite title with As a User, I want to create a Site for the first time and search title with As a user, I want to search documents, conversations, text blocks, and media. If status needs to be visible, use a metadata field or tag.

2. Document path contains "untitled" despite having a proper title

Problem: The document at path design/stories/authoring/untitled-CuOHmXgTTq has the title UT-13- Integrate document settings but its path still contains the auto-generated untitled- prefix with a random suffix. This path appears in embeds, query blocks, and the URL bar.

Why it matters: untitled- paths are opaque and unprofessional. They break the convention of descriptive, human-readable paths used throughout the rest of the design stories directory. The path also leaks the internal document ID suffix (CuOHmXgTTq) which is implementation detail.

Recommendation: Rename (move) the document to a descriptive path such as design/stories/authoring/integrate-document-settings or design/stories/authoring/ut-13-integrate-document-settings. Note: moving requires updating the embed reference in Stories.

3. Rapid public iteration continues (third consecutive week)

Problem: ⚠️As a User, I want to create a Site for the first time underwent 4 document updates on 2026-07-30 (within approximately 5 hours). The search design story also had 2 updates today. Each intermediate version is publicly visible and generates feed events. This pattern has been noted in the 2026-07-28, 2026-07-29, and now 2026-07-30 reports.

Recommendation: Use Seed's draft functionality for active iteration. Publish only when the iteration stabilizes. This is now a recurring pattern across three consecutive weeks.

Medium Priority

4. Design Work Status page is out of sync with actual story status

Problem: Design Work Status marks "Create a Site" as ✅ (done), but the actual design story createsite is still marked ⚠️ (in progress) and was actively edited today. The same applies to the Settings activity hub which marks UT-02 as complete.

Why it matters: Status drift between a tracker page and the actual documents creates confusion about what is truly done. Team members may make decisions based on stale status.

Recommendation: Either update the status tracker to reflect reality, or remove the tracker and rely on the embedded story documents as the single source of truth (since the Design hub already embeds stories with their status icons).

5. notes/layout-problems-and-improvements still lacks metadata tags

Problem: Flagged in the 2026-07-29 report: Layout Problems and Improvements still has no tags or summary metadata. It is a well-structured design critique document but remains undiscoverable via tag-based navigation.

Recommendation: Add tags: design, layout, ux, product. Add a brief summary describing the document's purpose.

6. links/old-notes embedded content may conflict with current design

Problem: As an Editor, I want to create and manage links in my documents embeds a sub-document Old notes containing older acceptance criteria that may conflict with the current design above it. The old notes contain detailed ACs for link creation and copy-paste behavior that duplicate the newer sections.

Why it matters: Having two sets of acceptance criteria in the same document (new on top, old embedded at the bottom) creates ambiguity about which criteria are authoritative.

Recommendation: Either mark the "Old notes" section with a clear banner ("⚠️ Historical — superseded by the criteria above") or move it to the Archived section.

7. Missing explicit backlink from PRD to its design story

Problem: PRD: Explore Knowledge is referenced from the search design story with the link text "PRD: Explore Knowledge", but the PRD itself does not link back to the design story. The PRD links out to external resources but has no backlink to its design implementation.

Why it matters: A reader discovering the PRD first has no way to find the corresponding design work. Bidirectional linking strengthens the knowledge graph.

Recommendation: Add a "Related Design" section at the bottom of the PRD linking back to the search design story.

Low Priority

8. Notes by Gabo has empty placeholder sections

Problem: Notes by Gabo — a sub-document embedded in the createsite story — contains empty sections (# Inspirations, # Projects, # Acceptance Steps, # Disputes) and duplicate lines ("Your first Site is free" and "Upload media up to 50mb per media file" appear twice). The document reads as rough brainstorming notes.

Why it matters: Empty sections and duplicated content reduce trust in the document's completeness. Since this is embedded as a Card in the createsite flow, readers see it as a finished artifact.

Recommendation: Clean up duplicates and either populate or remove empty sections. Consider renaming to Notes by Gabo (draft) to signal its work-in-progress nature.

9. New agentic accounts created with identical placeholder content

Observation: Three accounts were created or updated on 2026-07-30 with the identical home page content "This is an agentic account.": Ethos Assistant (two different accounts: z6MkoACrcu... and z6MkfEax...), and Importer (z6MkopuUd...). This follows the pattern noted in the 2026-07-29 report.

Note: External observation — not a Librarian repository issue. No action needed.

10. Design stories lack consistent metadata tags

Observation: Most design story documents under /design/stories/ have no tags metadata. Documents like As an Editor, I want to create and manage links in my documents do have good metadata (summary, cover, showOutline) but still no tags.

Recommendation: Add consistent tags across design stories: design, story, plus activity name (e.g., authoring, organization, settings, identity, conversations). This would enable tag-based filtering across the repository.

Positive Observations


    Cross-linking continues to improve: the createsite doc embeds Notes by Gabo and links to Make more space for content, the search story links to PRD: Explore Knowledge, and the Layout Problems doc links to the PRD as well.

    The Design hub page remains a well-maintained index with embedded activity sub-pages, status indicators, and links to all key sections (stories, research, misc, archived, pending, meetings).

    The Links design story has excellent metadata (summary, cover, showOutline, showActivity) — it could serve as a metadata template for other design stories.

    No orphan documents were detected in the recently modified set.

    No duplicate pages were found.

    The createsite/notes-by-gabo and links/old-notes sub-documents are properly embedded via Card/Embed rather than duplicated inline.
